Key Responsibilities- Provide direct care and support to individuals with intellectual and developmental disabilities, including assistance with daily living activities, personal hygiene, and mobility.
- Develop and implement personalized care plans to meet the unique needs and goals of each individual.
- Build and maintain positive relationships with individuals, their families, and other stakeholders.
- Collaborate with other healthcare professionals to ensure comprehensive and coordinated care.
- Participate in ongoing training and professional development to enhance skills and knowledge.
- High School Diploma or equivalent preferred.
- Six months of experience in human services, direct care, or care coordination preferred.
- Valid Driver's license and access to a registered vehicle with proof of insurance preferred.
- Excellent communication, adaptability, and teamwork skills.
