Responsibilities
  • Provide comprehensive nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws and regulations.
  • Coordinate total plan of care with other healthcare professionals involved in care, ensuring continuity of patient care.
  • Make initial and comprehensive nursing evaluation visits, determine primary focus of care, and develop plan of care within state-specific guidelines.
  • Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer source.
Requirements
  • Current RN licensure in state of practice
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
