Job Title: Registered Nurse - Hospice Care Coordinator

Location: The Pennant Group, Inc.

About the Role:

We are seeking a compassionate and skilled Registered Nurse to join our hospice care team as a Hospice Care Coordinator. As a key member of our interdisciplinary team, you will be responsible for providing high-quality patient care and coordinating services to meet the needs of individuals and families in their homes and communities.

Key Responsibilities:
  • Conduct comprehensive assessments of patients and families to determine their needs and develop personalized care plans
  • Collaborate with healthcare professionals to ensure seamless care coordination and communication
  • Provide education and support to patients and families on disease management, symptom control, and end-of-life care
  • Maintain accurate and detailed clinical records and reports
  • Participate in ongoing education and training to enhance skills and knowledge
Requirements:
  • Graduate of National League for Nursing accredited school of nursing
  • Current licensure to practice professional nursing in the state
  • Minimum 2 years of experience in public health, home care, or hospice nursing (preferably)
  • Demonstrated ability to work independently and prioritize tasks effectively
  • Excellent observation, problem-solving, verbal and written communication skills
Estimated Salary: $60,000 - $80,000 per year
