About the Role:

In this vital position, you will be responsible for delivering high-quality patient care and coordinating services to meet the needs of individuals and families in their homes and communities. Your expertise will be essential in ensuring that patients receive comprehensive, coordinated care that addresses their unique needs and preferences.

Key Responsibilities:
  • Conduct comprehensive assessments of patients and families to determine their needs and develop personalized care plans
  • Collaborate with healthcare professionals to ensure seamless care coordination and communication
  • Provide education and support to patients and families on disease management, symptom control, and end-of-life care
  • Maintain accurate and detailed clinical records and reports
  • Participate in ongoing education and training to enhance skills and knowledge
Requirements:
  • Graduate of National League for Nursing accredited school of nursing
  • Current licensure to practice professional nursing in the state
  • Minimum 2 years of experience in public health, home care, or hospice nursing (preferably)
  • Demonstrated ability to work independently and prioritize tasks effectively
  • Excellent observation, problem-solving, verbal and written communication skills
Estimated Salary: $70,000 - $90,000 per year
