RN Hospice Team Lead

We are looking for an experienced RN with a passion for delivering patient-centered care and a commitment to teamwork. The ideal candidate will possess current licensure as a Registered Nurse (RN) in the state and have a minimum of two years of experience in hospice or palliative care nursing. Prior management experience is not required, but the ability to supervise hospice aides and coordinate patient care is essential.
The salary range for this position is $90,000 - $115,000 per year, depending on experience, plus comprehensive benefits and opportunities for professional growth and development. The Key Responsibilities of this role include:
- Patient Care: Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs; provide physical assessments and histories of current and previous illnesses; utilize all elements of the nursing process to provide professional care.
- Communication: Maintain accurate and relevant clinical notes regarding patient conditions and care given; record pain/symptom management changes/outcomes; communicate with physicians and community health professionals to coordinate care plans.
- Additional Duties: Participate in on-call duties, ensure equipment and necessary items are available, and assume responsibility for personal growth and development through continuing education and in-service classes.
