Role Overview: Advanced Practice Nurse - In-Home Care

As an Advanced Practice Nurse, you will lead the charge in providing preventive care and educational services, engaging directly with members within their communities. This role offers the unique satisfaction of local travel and personal interactions with members in the comfort of their homes.

Your Responsibilities:

  • Conduct comprehensive assessments, including Annual Wellness and Quality Gap Care visits for members in their residences.

Location:

We are looking for candidates in the New Hampshire metro areas and require local applicants with reliable transportation.

Work Schedule:

  • Availability of 24-40 hours, Monday to Friday, from 8:00 AM to 5:00 PM.
  • Manage a caseload of 6-8 patients daily.
  • Each visit will last between 45 minutes to 1 hour.

Compensation Structure:

  • Starting compensation of $120 per completed visit.
  • Payment for same-day no-show visits.
  • Independent Contractor status.
  • Reimbursement for travel mileage.

Key Duties:

  • Perform clinical screenings and various assessments in an in-home environment.
  • Accurately document clinical findings and evaluations.
  • Utilize proprietary tools or client EMR for detailed medical and social assessments.
  • Identify and refer members for case management as needed.
  • Provide recommendations for follow-up with primary care providers.
  • Maintain compliance with HIPAA regulations regarding patient privacy.
  • Exhibit professionalism and courteous behavior with all members.
  • Ensure member satisfaction throughout the examination and visit.
  • Foster effective communication while maintaining member confidentiality.
  • Adhere to safety protocols and maintain a secure working environment.

Who You Are:

Our ideal candidate is deeply committed to delivering exceptional care. You possess a passion for continuous learning and self-improvement, with a meticulous approach to documentation that underscores the importance of comprehensive patient care. Additional qualifications include:

  • A minimum of 2 years of hands-on experience as an Advanced Practice Nurse.
  • Current, unrestricted licensure in the relevant state(s).
  • Board certification, preferably in geriatric or adult medicine.
  • Familiarity with HCC models and quality measures such as HEDIS and STAR.
  • Strong communication skills, particularly with elderly and chronically ill patients.
  • Proficiency in utilizing technology, including computers, tablets, and mobile devices.
  • Willingness to travel within the designated geographic area.
