Job Summary
The Community Care RN provides and supervises professional, independent nursing care for Providence patients. This role is based in the nursing process and is consistent with the Oregon Nurse Practice Act. The RN applies knowledge of assessing, planning, ordering, giving, delegating, teaching, and supervising professional nursing care delivery that promotes patients' optimal health and independence.
About the Role
Providence PACE is a Program of All-Inclusive Care for the Elderly that strives to keep older adults as healthy as possible living in the community through clinics, home visits, and more. As a Community Care RN, you will be part of a team that empowers elders in their community to live active, independent lives.
Responsibilities
- Assess, plan, and implement professional nursing care for patients
- Supervise and delegate tasks to other caregivers
- Teach patients and their families about health and wellness
- Collaborate with other healthcare professionals to provide comprehensive care
Requirements
- Oregon Registered Nurse License
- National Provider BLS - American Heart Association
- Relevant certifications applicable to the position
