Key Responsibilities- Assess, plan, and implement nursing care for patients, ensuring their needs are met and their health is optimized.
- Supervise and delegate tasks to other healthcare professionals, promoting a collaborative and effective care team.
- Develop and maintain relationships with patients, families, and caregivers, providing education and support to ensure optimal health outcomes.
- Collaborate with other healthcare professionals to coordinate care and ensure seamless transitions between settings.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices in care delivery.
- Current Oregon Registered Nurse License.
- National Provider BLS - American Heart Association certification within 30 days of hire.
- Relevant certifications applicable to the position, as specified by the Nurse Manager.
- Coursework/Training relevant to the position, as specified by the Nurse Manager.
- Bachelor's Degree in Nursing.
- 1 year of Home health RN experience.
