About the Role

We are seeking a highly skilled HCBS Wait List Care Coordinator to join our team at UnitedHealthcare. As a Care Coordinator, you will play a critical role in ensuring that individuals receive the necessary services and support to live healthy and fulfilling lives.

Key Responsibilities
  • Assess and develop individualized care plans that address the unique needs and goals of each person
  • Coordinate with healthcare providers, community organizations, and family members to ensure seamless care transitions
  • Identify and connect individuals with community resources and services that promote health and well-being
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes
  • Collaborate with interdisciplinary teams to develop and implement care strategies that address social determinants of health

Requirements
  • Bachelor's degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or related human services area
  • 1+ years of experience working with individuals with long-term care needs
  • Intermediate level of computer proficiency, including Microsoft Outlook and Teams
  • Ability to travel up to 75% of the time
  • Valid driver's license and reliable transportation
Preferred Qualifications
  • Licensed Social Worker or clinical degree
  • 1+ years of experience working with individuals receiving services on one of the home and community-based waivers in KS
  • Background in managing populations with complex medical or behavioral needs
