Job Summary

We are seeking a skilled LPN Assessment Nurse to join our Care Management Services team at CHSGa. As a key member of our team, you will play a vital role in ensuring the goals of our program and those of our members are met through primary care enhanced case management.

Key Responsibilities
  • Conduct thorough assessments of members in their home environment, gathering medical, environmental, and psychosocial data for review by our multidisciplinary team.
  • Accept referrals and ensure timely screening of potential members in accordance with organizational and governmental policies.
  • Address member or caregiver concerns effectively and efficiently, providing support and guidance as needed.
  • Collaborate with our multidisciplinary team to develop care plans, recommending HCBS services and incorporating roles for informal caregivers.
  • Record and document member information accurately and completely, adhering to federal, state, and operational guidelines.
  • Work to preserve the essential role of family and informal caregivers in assisting members in meeting care plan goals and addressing social risk.
  • Makes recommendations to our multidisciplinary team to address primary care needs.
  • Collaborate with our Quality Department to resolve items needing additional information or clarification, ensuring standards of promptness and member needs are met.
  • Advocate for informed decisions by members and caregivers regarding their health status and options for treatment or care.
  • Promote quality outcomes through effective collaboration with our Quality Assurance and Education Coordinator, Assessment Coordinator, and Administrator.
  • Utilize technology to promote workflow efficiency and facilitate data collection, management, and analysis.
Requirements
  • Valid Georgia LPN license with 2 years of experience in the health and human services field.
  • Experience in social work, home and community-based services, healthcare, or geriatrics preferred.
  • Valid Driver's License and reliable transportation.
