Job Summary:

We are seeking a highly skilled Clinical Pharmacist to join our team at Kadlec Regional Medical Center. As a Clinical Pharmacist, you will play a vital role in ensuring the optimal use of medications to achieve specific outcomes that improve patients' quality of life.

Key Responsibilities:

  • Work collaboratively with the interdisciplinary team to provide chronic disease state management to patients.
  • Initiate, modify, and discontinue medication therapy based on patient history, medical records, and barriers to access.
  • Monitor medication therapy for side effects, effectiveness, and allergies.
  • Document patient care processes appropriately within the electronic health record.
  • Provide drug information to patients and professional staff.

Requirements:

  • Bachelor's Degree from an accredited School of Pharmacy with a BS/PharmD degree or equivalent education/experience.
  • Washington State Pharmacist License upon hire.
  • One (1) year of PGY-1 residency or equivalent experience.

Preferred Qualifications:

  • Two (2) years of PGY-2 AmCare residency or equivalent experience.
  • Certification in Ambulatory Care upon hire.
