About the Role

Matrix Medical Network is seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, identifying potential health risks and providing personalized care.

Key Responsibilities:

  • Conduct thorough health assessments, including medical history, diagnosis, and treatment plans
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Provide education and support to patients and their families
  • Work in a variety of settings, including homes, skilled facilities, and telehealth

Requirements:

  • Master's degree in Nursing or related field
  • Current RN and NP licensure, with prescription authority
  • Board certification in a Matrix-approved specialty
  • 1+ year of experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
