We are seeking a highly motivated and organized Care Coordinator to join our Lewiston Children's BHH team. The successful candidate will provide essential support and coordination services to families within our catchment area.

Compensation and Benefits

The estimated annual salary for this role is $43,200 - $57,600, paid bi-weekly at $21.00-$27.00 per hour. Key Responsibilities

The Care Coordinator will be responsible for:

  • Collaborating with providers to develop and implement individualized treatment plans;
  • Providing ongoing support and guidance to families and caregivers;
  • Completing thorough assessments and documenting progress;
  • Staying up-to-date with relevant laws, regulations, and best practices in the field.
Requirements
