**Job Overview**
We are seeking a highly skilled Healthcare Navigator to join our team. As a key member of our care management team, you will play a critical role in delivering exceptional patient-centered care to our members.
**About the Role**
The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for working with patients and families to navigate complex healthcare systems.
**Key Responsibilities:**
- Develop and implement personalized care plans that address the unique needs and goals of each patient
- Provide education and support to patients and families on health conditions, treatment options, and self-care strategies
- Facilitate effective communication between patients, families, and healthcare providers
- Collaborate with multidisciplinary teams to coordinate care and ensure seamless transitions
- Monitor patient progress and adjust care plans as needed
**Requirements:**
- Bachelor's degree in Nursing (BSN) or related field
- Current CCM Certification
- Minimum 5 years of experience in nursing, preferably in care management or case management
- Excellent communication and interpersonal skills
- Ability to work independently and collaboratively as part of a team
- Strong analytical and problem-solving skills
**What We Offer:**
