Job Overview

Position: Outreach Care Specialist

Work Environment: This role is primarily field-based, requiring candidates to engage actively within the community. Candidates must reside in Connecticut to be eligible for consideration.

Work Schedule: The standard working hours are Monday to Friday, from 9 AM to 5 PM. Flexibility to accommodate the needs of families is essential, and working outside these hours may be necessary.

The Outreach Care Specialist is responsible for providing support to individuals and families, ensuring they are connected to community resources, services, and care coordination. This role also emphasizes effective communication among the care team and treating clinicians to support members and their families. The specialist will coordinate member-specific care plans within the care network and collaborate with ICC staff, families, community partners, stakeholders, and providers to ensure that designated members receive appropriate services. The primary objective is to enhance independence and quality of life by assessing individual needs and facilitating access to community services and supports.

Key Responsibilities:

  • Coordinate follow-up care plans for members by scheduling appointments or enrolling them in relevant programs.
  • Assess member adherence to medical treatment plans through phone calls or on-site visits.
  • Identify barriers to compliance and work towards resolving them.
  • Recognize opportunities that impact quality goals and suggest process enhancements.
  • Recommend modifications to treatment plans and determine the need for additional services in collaboration with case management and providers.
  • Facilitate identification and referral to local, state, or federally funded programs.
  • Coach members on strategies to mitigate health risks.
  • Prepare documentation to track case and compliance updates.
  • Establish and maintain relationships with relevant agencies as per contract requirements.
  • Perform other duties as assigned.

Minimum Qualifications:

  • A high school diploma and at least 1 year of relevant experience, or a combination of education and experience that provides an equivalent background.

Preferred Skills and Experience:

  • Familiarity with community resources in the area.
  • Bilingual candidates (Spanish) are preferred.
  • A BS/BA degree in a related field is preferred.
  • Experience with electronic health records (EHR) systems.
  • Strong oral, written, and interpersonal communication skills, along with problem-solving, facilitation, and analytical skills are expected for this role.
