Job Title: Senior Care Coordinator

In this role, you'll work with residents and families to create personalized care plans based on comprehensive assessments. You'll also collaborate with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities:

  • Develop and implement care plans tailored to individual resident needs
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Conduct regular resident assessments and updates

Requirements:

  • Bachelor's degree in nursing or related field
  • One year of professional nursing experience in skilled nursing settings
  • Strong communication and interpersonal skills
