About the Role

The University of New Mexico - Hospitals seeks a skilled and compassionate Registered Nurse to join our Case Management team as a Multidisciplinary Care Coordinator. This dynamic role requires a strong ability to coordinate patient care, communicate effectively with healthcare teams, and ensure quality patient outcomes.

Key Responsibilities

  • Coordinate all aspects of patient care, ensuring seamless transitions between departments and services.
  • Collaborate with physicians, nurses, and other healthcare professionals to develop comprehensive treatment plans.
  • Conduct regular assessments to identify patient needs and develop targeted interventions.
  • Maintain accurate and timely documentation of patient progress and care plans.
  • Communicate effectively with patients, families, and caregivers to ensure informed decision-making and empowered participation in care.

Requirements

  • Current RN licensure in New Mexico or eligibility for reciprocal agreement.
  • 2 years of direct experience in nursing, preferably in case management or a related field.
  • Bilingual English/Spanish language proficiency highly desirable.
  • CPR for Healthcare/BLS certification within 30 days of hire.
