**Job Summary:**

We are seeking a skilled and dedicated Registered Nurse to join our team as a Senior Patient Care Coordinator RN in our Per Diem capacity. As a vital member of our healthcare team, you will be responsible for coordinating patient care, ensuring quality outcomes, and providing exceptional patient experience.

**Key Responsibilities:**

  • Coordinate patient care services, ensuring efficient utilization of resources and quality outcomes.
  • Develop and implement individualized treatment plans, collaborating with healthcare teams to achieve patient-centered goals.
  • Conduct ongoing assessments, analyze data, and identify opportunities for improvement to optimize patient care.
  • Maintain accurate and timely documentation, ensuring seamless communication among healthcare teams.

**Requirements:**

  • 2 years of direct experience in nursing, preferably in case management or related field.
  • RN licensure in New Mexico or reciprocal agreement.
  • CPR for Healthcare/BLS Provider certification within 30 days.
