Care Coordinator for Veterans

We are seeking a dedicated and compassionate Care Coordinator to join our team at the Veterans Affairs, Veterans Health Administration. In this role, you will play a vital part in ensuring the well-being of our nation's veterans by coordinating their care and providing support throughout their healthcare journey.
About the JobThis position is located at the New Mexico VA Healthcare Network, where you will be working closely with our interdisciplinary treatment teams to provide comprehensive care to veterans with complex medical and psychiatric needs. Your expertise in social work and ability to assess and treat psychosocial problems will be invaluable in helping our veterans achieve their goals and improve their quality of life.
Responsibilities- Work with veterans and their families to identify and address their unique challenges and needs.
- Develop and implement personalized care plans that take into account each veteran's specific circumstances and goals.
- Collaborate with healthcare providers, family members, and community resources to ensure seamless care coordination and efficient communication.
- Provide education and support to veterans and their families on topics such as community resources, VA benefits, and advance directives.
To be successful in this role, you must possess:
- A Master's degree in Social Work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE).
- Experience working with diverse populations, including veterans with complex medical and psychiatric needs.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
- Familiarity with community resources and VA benefits.
