Lead Case Manager, Behavioral Health Services Coordinator

Job Summary: We are seeking a highly skilled and compassionate Lead Case Manager to join our Behavioral Health team at Catholic Charities USA. As a key member of our team, you will provide case management and supportive services for children, adults, and older adults, ensuring quality service delivery and effective interdepartmental workflow strategies.

Key Responsibilities:
  • Assume overall responsibility for the day-to-day lead of the Drop-In Center services, ensuring quality service delivery and continuous quality assurance/improvement.
  • Facilitate and co-facilitate drop-in services groups in the community, assessing and directing continuous quality assurance/improvement to ensure quality and effectiveness of service delivery.
  • Review and approve the psychoeducational, recreational, and social skills curriculum to determine the most appropriate presentations.
  • Oversee and ensure timely data entry, providing statistics to Santa Clara County program contract monitor every month.
  • Communicate and collaborate with the Program Manager, Senior Clinical and Wellness Division Director, and other department managers to ensure effective interdepartmental workflow strategies.
  • Participate in recruitment, training, supervision, and evaluation of case managers to ensure the success, sustainability, resourcefulness, and ongoing development of program services, support, healing methods, and evidence-based practices.
  • Participate in annual staff performance evaluations and goal-setting, assess staff needs, and schedule training accordingly.
  • Provide strong leadership and people management that reflects Catholic Charities' values, including mutual respect, compassion, and integrity.
  • Ensure that all program contract requirements are met and regular contact with the appropriate county contacts is made.
  • Attend all collaborative county/city and other program meetings.
  • In collaboration with the Program Manager, develop and implement new supportive programming.
Requirements:
  • Bachelor's degree in social work, human services, counseling, or equivalent combination of education and experience.
  • Minimum of two (2) years experience in human services or Behavioral Health.
  • Experience in office and administrative processes, procedures, and program development.
  • Experience in training staff and volunteers is preferred.
Preferred Qualifications:
  • Bilingual in Spanish/Vietnamese/Mandarin.
  • Knowledge of community resources, organizations, and supports.
  • Knowledge of mental health and alcohol, substance abuse, and recovery treatment services.
Working Conditions:
  • Willing to go to clients or community agencies as necessary through own vehicle or public transport.
  • Some travel within Santa Clara County for client services. Some travel between sites for program needs.
