Job Summary

We are seeking a skilled Healthcare Coordinator to join our team at HMSA. As a key member of our healthcare team, you will be responsible for coordinating care for our members, ensuring they receive timely and appropriate medical care and services.

Key Responsibilities
  1. Conduct Assessments and Develop Care Plans: Conduct timely face-to-face assessments and develop targeted health action plans/care plans for members requiring assistance with care coordination.
  2. Coordinate Care Teams: Coordinate a multi-disciplinary team to develop person-centered care plans, including members' primary care physicians, other service providers, and family members/caregivers.
  3. Document and Resolve Issues: Document and resolve issues for eligible HMSA members and providers on electronic systems.
  4. Ensure Continuity of Care: Provide continuity of care when members are discharged from hospitals to avoid re-admissions.
  5. Coordinate with External Agencies: Coordinate and monitor activities among internal departments, external community agencies, and HMSA partners/vendors.
  6. Utilize Data to Inform Care: Utilize compiled data from member encounters to ensure services provided meet members' needs.
  7. Assist with Policy Development: Assist manager in updating or establishing new policies and procedures as needed.
  8. Provide Support to Members and Providers: Provide assistance in resolving concerns about service delivery or providers.
