Key Responsibilities- Facilitate lifelong partnerships with patients to equip and empower them to access resources and adopt healthy lifestyles.
- Assess patient needs, plan, coordinate, and evaluate services to achieve timely and appropriate patient management.
- Collaborate with multidisciplinary care teams, including providers, pharmacists, social workers, behavioral health specialists, and nurses to achieve optimal outcomes.
- Utilize the RN process as a framework to focus healthcare team activities on achievement of optimal outcomes, resource utilization, clinical expertise, and improvement strategies.
- Interact with patients, professionals, and communities to achieve continuity of care, coordination of services, and documentation of plans across multiple care settings.
- Conduct or participate in comprehensive needs assessments for identified patients, knowledgeable of appropriate care-related services to match identified needs.
- Maintain accurate case records of each referred customer/patient and document fully and accurately.
This role requires:
- Bachelor's degree in Nursing (BSN) required; Master's preferred.
- Current RN license or temporary license as a Registered Nurse Petitioner in the state where you work and reside.
- Two years' experience required in healthcare, including case management/discharge planning in acute care, home care, LTC care, physician office, or managed care company.
- Appropriate professional certification required within 3 years of hire date for professional certification per departmental protocol.
$64,000 - $84,000 annually, depending on experience and location.
