Atrium Health is seeking a dedicated and compassionate RN Care Manager to join our team. As a RN Care Manager, you will play a vital role in enhancing and personalizing the management of health-related needs for our patients.

Key Responsibilities
  • Facilitate lifelong partnerships with patients to equip and empower them to access resources and adopt healthy lifestyles.
  • Assess patient needs, plan, coordinate, and evaluate services to achieve timely and appropriate patient management.
  • Collaborate with multidisciplinary care teams, including providers, pharmacists, social workers, behavioral health specialists, and nurses to achieve optimal outcomes.
Essential Functions
  1. Utilize the RN process as a framework to focus healthcare team activities on achievement of optimal outcomes, resource utilization, clinical expertise, and improvement strategies.
  2. Interact with patients, professionals, and communities to achieve continuity of care, coordination of services, and documentation of plans across multiple care settings.
  3. Conduct or participate in comprehensive needs assessments for identified patients, knowledgeable of appropriate care-related services to match identified needs.
  4. Maintain accurate case records of each referred customer/patient and document fully and accurately.
Requirements and Qualifications

This role requires:

  • Bachelor's degree in Nursing (BSN) required; Master's preferred.
  • Current RN license or temporary license as a Registered Nurse Petitioner in the state where you work and reside.
  • Two years' experience required in healthcare, including case management/discharge planning in acute care, home care, LTC care, physician office, or managed care company.
  • Appropriate professional certification required within 3 years of hire date for professional certification per departmental protocol.
Estimated Salary Range

$64,000 - $84,000 annually, depending on experience and location.
