We are seeking a highly skilled and dedicated Nurse-Family Partnership Supervisor to join our team at Ascend Nonprofit Solutions. The successful candidate will have the opportunity to make a meaningful impact in the lives of families by providing supervision, guidance, and support to nurse home visitors.

Job Description

The Nurse-Family Partnership Supervisor will be responsible for ensuring the effective implementation of the Nurse-Family Partnership program within our organization. This will involve supervising and mentoring nurse home visitors, developing and maintaining community referral sources, and building relationships with team colleagues and community members.

Key Responsibilities

  • Supervise and mentor nurse home visitors to ensure they have the necessary skills and knowledge to provide high-quality care to families.
  • Develop and maintain community referral sources to ensure a steady flow of referrals to the program.
  • Build and maintain relationships with team colleagues and community members to promote collaboration and cooperation.
  • Ensure that supervision of nurse home visitors utilizes principles and concepts of motivational interviewing, coaching, and reflective practice.
  • Conduct regular field visits and evaluations of nurse home visitors and administrative support staff.
  • Provide regular case conferences and team meetings as recommended by NFP.
  • Evaluate implementing agency outcomes and interventions.
  • Provide developmental feedback and address performance issues with staff.

Administrative Skills

  • Oversee the recruitment and hiring of nurse home visitors and administrative support staff.
  • Ensure that all staff attend required NFP education sessions.
  • Oversee and ensure timely and accurate data input and utilization of web-based reports.
  • Read and interpret reports, sharing appropriately with team and partners.
  • Participate in, chair, or coordinate NFP community coalition/advisory board meetings.
  • Develop and implement external and internal systems to assure timely receipt and disposition of referrals.

Clinical Skills

  • Build confidence and skill of nurse home visitors.
  • Provide adequate orientation, professional development opportunities, and study time to remediate gaps in necessary knowledge and skill.
  • Effectively assist nurse home visitors in analyzing complex problems and case situations.
  • Structure appropriate continuing education and guidance of staff.
  • Ensure implementation of quality improvement strategies.

Qualifications and Education Requirements

  • Master's Degree in Nursing or related field required.
  • Valid unencumbered NC or SC RN license required.
  • Covid-19 vaccination and booster required as a condition of employment.
  • Two years recent experience in maternal/child health, public health, home visiting, mental/behavioral nursing, or working with diverse populations preferred.
  • Excellent computer skills necessary, including MS Office Suite applications or equivalent programs.
  • Strong interpersonal skills required to communicate effectively in both written and oral forms, including presentations.
  • Valid driver's license and car insurance required.
  • Current CPR required.
  • Bilingual Spanish preferred.
  • Ability to exercise independent judgment and reason objectively.
  • Experience in managing multi-disciplinary team members.

Position Requirements

  • Regularly sit, use hands to finger, handle or feel, and talk or hear.
  • Specific vision abilities required include close vision, distance vision, color vision, depth perception, and ability to adjust focus.

  • Travel may include local meetings, home visit supervision, community meetings, and occasional overnight travel to meetings or conferences.

Salary Range

$60,000 - $80,000 per year, depending on experience.
