Job SummaryThe RN Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive/complex/safe discharge plans. Collaborates with MSW for guidance with psychosocially complex cases. The RN Case Manager is also responsible for facilitating coordination of care. This shall be in accordance with the current policies and procedures, mission, vision, and values of Novant Health.
Serves as a resource for discharge planning and care coordination for medically complex cases. Maintains current knowledge of available community resources/post acute care options.
Your Responsibilities- Deliver the most remarkable patient experience in every dimension, every time.
- Collaborate with physicians and other healthcare professionals to develop and implement effective discharge plans.
- Facilitate coordination of care to ensure seamless transitions for patients.
To be successful in this role, you will require:
- An Associate Degree in Nursing or BSN (required) or 4-Year/Bachelor's Degree (preferred).
- Two or more years of healthcare experience (preferable), with case management or related experience.
- Current RN licensure in the state of employment (required).
- Ability to work weekends and holidays.
- Knowledge of Microsoft Office products and keyboarding skills (desired).
We offer a competitive salary and benefits package, estimated at $65,000 - $85,000 per year, depending on location and experience.
