We are seeking a skilled and compassionate Registered Nurse to join our team at Providence Home and Community Care (HCC). As a Registered Nurse, you will play a vital role in providing high-quality patient care in the comfort of their own homes.
About the Role:This is a full-time evening shift position, with one weekend rotation per month. You will be traveling around North Orange County regions, working alongside an interdisciplinary team to provide whole-person quality care to patients in need.
Responsibilities:- Provide skilled nursing care to patients in their homes, including wound care and use of negative pressure devices.
- Work collaboratively with the interdisciplinary team to develop and implement individualized care plans.
- Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address these needs.
- Administer medications and treatments as prescribed by physicians, and monitor patients' responses to these interventions.
- Document patient progress and communicate with the healthcare team to ensure seamless care.

To be successful in this role, you will need:
- 1 year of professional nursing experience.
