As a Clinical Care Coordinator, you will play a vital role within an Integrated Care Model team, addressing both medical requirements and social determinants of health. Your responsibilities will include conveying essential information regarding members' needs, advocating for optimal care solutions, and ensuring access to appropriate services tailored to individual requirements.

This role is primarily field-based with a home office setup.

Key Responsibilities:

  • Evaluate, strategize, and implement personalized care plans for members, focusing on the most suitable and least restrictive levels of care.
  • Oversee the care plan throughout the entire continuum, serving as a consistent point of contact.
  • Facilitate communication among all parties involved to ensure timely and quality coordinated care and services for all members.
  • Act as an advocate for patients and their families, ensuring their needs and preferences are fully represented and supported by the health care team.
  • Anticipate spending approximately 50-80% of your time in the field, visiting members in their residences or long-term care facilities.

Your performance will be recognized and rewarded in an environment that challenges you while providing clear guidance on achieving success in your role, along with opportunities for development in other areas of interest.

Required Qualifications:

  • Current, unrestricted RN license in MA with a minimum of 2 years of clinical experience.
  • At least 2 years of experience in long-term care, home health, hospice, community health, or assisted living settings.
  • Proficiency in MS Word and Outlook, with a minimum of 2 years of experience.
  • Willingness to travel within the assigned region to visit members in various settings.
  • Access to high-speed internet at home.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN) or currently pursuing a BSN.
  • Experience with electronic health records.
  • Prior field-based work experience.
  • Background in managing populations with complex medical or behavioral health needs.
  • Bilingual capabilities.
