Salary Range: $55,000 - $65,000 per annum

Job Description:

We are looking for a highly motivated and organized individual to provide social care services to patients and their families. As a Social Care Coordinator - Patient Services, you will play a vital role in assisting patients and their loved ones with personal and environmental difficulties that may impact their health and well-being.

Key Responsibilities:

  • Develop and implement social care plans to meet the unique needs of each patient
  • Collaborate with healthcare professionals to provide emotional support and advocacy to patients and their families
  • Assist with the completion of Minimum Data Set (MDS) assessments and interviews
  • Stay up-to-date with the latest community resources and services to ensure effective referrals and connections
  • Communicate effectively with patients, families, healthcare professionals, and community agencies to ensure seamless care coordination

Required Skills and Qualifications:

To be successful in this role, you will need:

  • A Bachelor's degree in social work from an accredited institution
  • At least one year of experience in a healthcare setting, preferably in social work or a related field
  • icensure as a Licensed Social Worker (LSW) in the state of Mississippi
  • Basic Life Support certification
