The MDS Coordinator plays a vital role in ensuring the delivery of high-quality patient care. As a key member of the care team, this individual is responsible for coordinating and directing activities related to patient care, while also developing and implementing policies and procedures to support these efforts.

Maintaining operational aspects of the patient care unit in collaboration with the Administrator, the MDS Coordinator is accountable for the completion and coordination of the MDS and interdisciplinary care plan on all patients admitted to the Transitional Care Unit.

Key responsibilities include:

  • Coordinating and directing patient care activities
  • Developing and implementing policies and procedures
  • Maintaining operational aspects of the patient care unit
  • Ensuring the completion and coordination of the MDS and interdisciplinary care plan

Requirements include:

  • 2-year associate's degree in Nursing
  • SC RN license and BLS certification
  • 1-3 years' experience in a healthcare setting
