Job Summary:

The MDS Coordinator is responsible for assisting the MDS Nurse in administering, directing, and coordinating activities of Patient Care. This role involves recommending, developing, and implementing policies and procedures in these areas, as well as keeping the ACNO informed of progress and potential problems.

Key Responsibilities:

  • Direct and evaluate patient care delivery
  • Maintain operational aspects of the patient care unit in collaboration with the Administrator
  • Responsible for completion and coordination of the MDS and interdisciplinary care plan on all patients admitted to the Transitional Care Unit

Requirements:

  • 2-year associate's degree required
  • SC RN license and BLS required
  • 1-3 years' experience preferred
