Position Overview:
We are seeking a dedicated Rehabilitation Services Coordinator who is a licensed PT, OT, SLP, or LCSW. This role is essential in providing coordination services for our rehabilitation team, ensuring the highest standards of care.

Key Responsibilities:
+ Oversee the delivery of personalized patient services and collaborate with clinical staff to create effective rehabilitation plans.
+ Develop and implement rehabilitation treatment strategies, setting functional outcomes and goals in partnership with patients and their support systems.
+ Prepare initial, progress, and discharge reports, continuously monitoring patient status.
+ Regularly assess satisfaction levels of patients, families, and healthcare providers regarding the rehabilitation program.
+ Organize interdisciplinary case conferences involving clinical teams, payers, and family members to discuss patient progress.
+ Maintain open communication with patients, families, and healthcare providers regarding treatment updates and any arising issues.
+ Facilitate the procurement of necessary equipment to support patient goals, working with funding sources as needed.
+ Participate in meetings with medical providers to address and resolve any clinical concerns related to the caseload.
+ Monitor service hours delivered to patients and report any changes to management.
+ Negotiate with funding sources for service extensions beyond initial authorizations.
+ Ensure compliance with company policies, HIPAA regulations, and professional standards.
+ Engage in ongoing professional development and training to enhance clinical skills.

Qualifications:
+ Bachelor’s degree in Allied Health (OT, PT, SLP), Nursing, Human Services, or Rehabilitation Counseling/Psychology.
+ Current professional licensure/registration/certification in the state of practice.
+ Certification as a Certified Brain Injury Specialist (CBIS) is preferred.
+ Minimum of two years of experience in a rehabilitation environment, particularly with neurologically impaired patients.
+ Experience in case management or insurance administration is advantageous.
+ Strong organizational, problem-solving, and communication skills are essential.
+ Ability to work independently and manage multiple tasks effectively.
+ Reliable transportation for travel to various assignments.
