Job Summary

We are seeking a skilled Healthcare Coordinator to join our team at Los Altos Sub-Acute and Rehabilitation Center. As a key member of our interdisciplinary team, you will play a vital role in coordinating the delivery of rehabilitation services to our patients.

Key Responsibilities
  • Coordinate the delivery of rehabilitation services to patients in collaboration with facility staff.
  • Monitor and document the cost-effectiveness of treatments provided, facilitating admission and discharge processes.
  • Serve as a patient advocate and liaison to insurance carriers and other medical management professionals.
  • Evaluate and communicate resident status, changes in functions, and care plans to payers.
  • Document all payer interactions regarding resident progress, expected outcomes, and special instructions.
  • Meet with the interdisciplinary team to coordinate services, ensuring residents' total regimen of care is maintained.
  • Coordinate and communicate with the business office and medical records to ensure accurate census and payment of managed care and Medicare residents.
Requirements
  • Current California LVN or RN license in good standing.
  • Minimum 3 years of experience in a hospital, nursing, or other related healthcare facility.
  • Working knowledge of managed care rules, regulations, and guidelines.
  • Outstanding communication (oral and written) skills.
  • Ability to multi-task and treat everyone at work kindly and professionally.
  • Works collaboratively and independently, following procedures and guidelines.
  • Completes tasks on time, reporting on successes, challenges, and offering solutions.
  • Emotional maturity – ability to remain calm and professional under stress.
