Role Summary

We are currently in search of a full-time Care Coordinator to deliver care coordination and supportive services to our clients.

The Care Coordinator will employ an intensive case management approach to connect clients with essential services such as childcare, transportation, housing, employment, and education, assisting them in overcoming barriers to long-term recovery.

Key Responsibilities
  • Review medical and mental health assessments for all assigned clients, ensuring necessary connections and follow-ups with providers.
  • Engage individually with clients throughout their treatment journey, developing tailored work plans to address and remove obstacles while documenting progress.
  • Collaborate with clinical staff to identify and address client needs effectively.
  • Establish and maintain strong working relationships with various community providers and resources within the service area.
  • Offer support to clients requiring assistance with childcare, housing, benefit applications, job searches, and other stabilization services.
  • Assist in scheduling appointments and making referrals to community resources as necessary.
  • Document all interactions in the electronic records system, maintaining records to track referrals and the impact of community resource engagement.
  • Promote treatment programs and gather materials for presentation to community partners.
  • Complete required training, including certification in data programs, case management, and SOAR.
  • Participate in planning and information meetings with other Care Navigators.
  • Uphold strict confidentiality in accordance with HIPAA and 42CFR regulations.
Qualifications

Qualified candidates should possess at least one year of experience in a human services role. A valid driver's license and the ability to pass a comprehensive background check are also required.
