We are currently seeking a full-time Care Coordinator to deliver care coordination and supportive services to patients in need.
Key Responsibilities- Utilize an intensive case management approach to connect patients with essential services, including childcare, transportation, housing, employment, and educational resources.
- Conduct thorough reviews of medical and mental health assessments for all assigned patients, ensuring necessary connections and follow-ups with healthcare providers.
- Engage with patients individually throughout their treatment journey, developing tailored work plans to address and eliminate barriers while tracking their progress.
- Collaborate closely with clinical staff to identify and address patient needs effectively.
- Establish and maintain strong partnerships with various community providers and resources within the service area.
- Assist patients in accessing support for childcare, housing, benefit applications, job searches, and other stabilization services.
- Document all interactions in the electronic records system, maintaining accurate records to monitor referrals and the impact of community resource participation.
- Promote treatment programs and gather materials for presentation to community partners.
- Complete mandatory training, including certifications in data programs, case management, and SOAR.
- Participate in planning and informational meetings with fellow Care Navigators.
- Uphold strict confidentiality in compliance with HIPAA and 42CFR regulations.
Qualified candidates should possess a minimum of one year of experience in a human services role. A valid driver's license and the ability to pass a comprehensive background check are also required.
