Position Overview:
The Healthcare Coordination Specialist plays a vital role in managing and facilitating care for patients within the acute inpatient setting.

This position requires collaboration with attending physicians, nursing staff, patients and their families, utilization review teams, and other healthcare professionals to ensure a seamless continuum of care that optimizes both clinical outcomes and cost efficiency.

The Specialist is responsible for liaising with community healthcare providers and case managers to ensure appropriate care transitions, aiming for safe discharges and minimizing the risk of readmission.


Key Responsibilities:
Conduct a comprehensive face-to-face assessment for each admitted patient to identify discharge requirements within 24 hours of admission.

Record findings in the electronic health record and communicate these outcomes to formulate an effective transition plan in conjunction with the healthcare team and the patient/family.

Actively participate in daily care rounds, providing updates on patient progress towards established discharge goals.
Review and document discharge plans in accordance with organizational policies and regulatory standards.
Maintain daily documentation of care progression, updates, and any obstacles to discharge for assigned patients.
Ensure timely management of care progression in collaboration with physicians and nursing staff to avoid unnecessary delays.
Work closely with Social Workers to coordinate long-term care options, financial assistance, and other necessary services.

Facilitate referrals and communication with in-network providers for post-acute care services such as Home Health, Durable Medical Equipment, IV infusion, Skilled Nursing Facilities, and Rehabilitation.

Promptly identify, document, and escalate any delays in care or discharge barriers, proposing solutions to rectify these issues and recognizing systemic trends that may require corrective measures.

Ensure follow-up appointments and referrals to case managers for high-risk patients are scheduled and communicated to patients and their families prior to discharge.

Adhere to all safety protocols and regulatory requirements within the facility.
Engage in Quality Assurance initiatives and the implementation of programs aimed at enhancing care quality indicators.
Exhibit professionalism in all responsibilities as directed by the designated supervisor.
Foster cooperative relationships with patients, their representatives, facility staff, physicians, consultants, and ancillary service providers.

Basic Qualifications:
Experience

A minimum of one (1) year in an acute care, medical/surgical, emergency department, or critical care nursing environment.

Education

An Associate's degree in Nursing is required.

Licensure, Certification, Registration

This position requires credentials from multiple states. Credentials from the primary work state must be obtained prior to hire, with additional credentials from secondary states required post-hire.

Registered Nurse License (Maryland) within 6 months of hire AND Registered Nurse License (Virginia) within 6 months of hire

OR

Compact License:
Registered Nurse within 6 months of hire

Registered Nurse License (District of Columbia) within 6 months of hire

Basic Life Support certification

Additional Requirements:
Experience with electronic medical record systems
Familiarity with healthcare reimbursement systems (Medicare Advantage, Commercial payers, Medicaid, and CMS regulations)

Annually:
Successful completion of the PCC Assessment of Critical Skills, with a passing score on inter-rater reliability.
Ability to communicate effectively with physicians, patients, and their families, as well as hospital staff.
Capability to work independently while applying critical thinking for problem-solving and decision-making.
Adherence to organizational policies and procedures.
Strong time management skills to develop organized workflows in a high-volume environment with rapidly changing priorities.
Intermediate proficiency in computer skills (Microsoft Office Suite).
Ability to manage and cope with ambiguity.
Ability to promote teamwork and work effectively as part of a team.
Excellent verbal and written communication skills.
Ability to interact effectively with key internal and external stakeholders.
Deliver exceptional customer service to enhance the patient experience.

Preferred Qualifications:
Recent experience in acute care, case management, or home health is preferred.
Bachelor of Science in Nursing (BSN) is preferred.
Experience in inpatient discharge planning is preferred.
