Position Overview

Role Summary

We are currently seeking a full-time Care Coordinator to deliver care coordination and supportive services to our patients.

Key Responsibilities
  • Conduct thorough reviews of medical and mental health assessments for all assigned patients, ensuring appropriate connections and follow-ups with healthcare providers.
  • Engage with patients on an individual basis throughout their treatment journey, developing tailored work plans to eliminate barriers and documenting their progress.
  • Collaborate closely with clinical staff to identify and address the needs of patients.
  • Establish and nurture strong relationships with community providers and resources within the service area.
  • Assist patients in accessing essential services such as childcare, housing, benefit applications, job searches, and other stabilization support.
  • Schedule appointments and make referrals to community resources as necessary.
  • Maintain accurate documentation of all interactions in the electronic records system, tracking referrals and patient participation in community resources.
  • Promote treatment programs and gather materials for presentation to community partners.
  • Complete all required training, including certifications in data programs, case management, and SOAR.
  • Participate in planning and informational meetings with fellow Care Navigators.
  • Uphold strict confidentiality in accordance with HIPAA and 42CFR regulations.
Qualifications

Ideal candidates will possess a minimum of one year of experience in a human services role. A valid driver's license and the ability to pass a comprehensive background check are also required.
