Responsibilities:
  • Coordinate case management services, collaborating with multidisciplinary teams to deliver holistic patient care.
  • Develop and maintain relationships with community service providers, fostering partnerships that enhance patient outcomes.
  • Conduct thorough assessments, identifying patient needs and developing personalized care plans.
  • Communicate effectively with patients, families, and healthcare teams, ensuring seamless coordination of care.
  • Maintain accurate records and reports, utilizing EHRs and other documentation tools.
Requirements:
  • A minimum of 2-3 years' experience in behavioral health or a related field.
  • Strong communication and interpersonal skills, with the ability to build rapport with diverse populations.
  • A High School Diploma is required, with an Associate's Degree preferred.
