Job Summary

We are seeking a skilled and compassionate RN Case Manager to join our team at Community Hospice & Palliative Care. As a RN Case Manager, you will play a vital role in providing high-quality patient care and support to individuals and their families in need of hospice and palliative care services.

Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of patients to identify their physical, emotional, and social needs, and develop individualized care plans to address these needs.
  • Patient Care Coordination: Coordinate and provide patient care services, including skilled nursing, medication management, and emotional support, in accordance with physician orders and patient goals.
  • Documentation and Communication: Maintain accurate and timely documentation of patient care services, and communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Team Collaboration: Collaborate with interdisciplinary teams, including physicians, social workers, and spiritual care specialists, to ensure comprehensive patient care and support.
  • Quality Improvement: Participate in quality improvement initiatives to enhance patient care and services, and identify opportunities for process improvements.
Requirements
  • Education: Possession of a degree from an accredited school of nursing program, such as an Associate Degree or Bachelor's Degree in Nursing.
  • Experience: One year of Registered Nurse experience, preferably in hospital medical-surgical or oncology settings, or five years of Licensed Practical Nurse experience within the past five years, with one year of hospital medical-surgical or oncology experience preferred.
  • Skills: Effective communication and teamwork, knowledge of hospice and palliative care principles and methods, and ability to manage inpatient facility functions and patient care needs.
