About the Role

We are seeking a highly skilled and compassionate Nurse Practitioner - Primary Care Specialist to join our team at Catholic Health Initiatives. As a key member of our primary care team, you will be responsible for providing high-quality, patient-centered care to our patients.

Key Responsibilities
  • Provide comprehensive primary care services to patients, including health assessments, diagnosis, and treatment of acute and chronic conditions.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Stay up-to-date with the latest medical research, guidelines, and best practices to provide evidence-based care.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
  • Master's degree from an accredited Nurse Practitioner program.
  • Current licensure as a Nurse Practitioner in the state of Texas (or ability to obtain).
  • National Board Certification as a Nurse Practitioner.
  • Basic Life Support (BLS) certification.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced, dynamic environment.
