The Nursing Admissions Coordinator (RN/LPN) plays a vital role in supporting the Director of Health Services and Assistant Director of Health Services in planning, organizing, developing, and overseeing the admission intake and medical records functions of the Nursing Department(s). This is done in accordance with current federal, state, and local standards, guidelines, and regulations that govern our health campus, while ensuring the highest quality of care is consistently maintained.


WHAT WE'RE LOOKING FOR

Here's what you'll do when you join us:

  • Collaborate with the Director of Health Services (DHS) and Assistant Director of Health Services (ADHS) to plan, develop, organize, implement, evaluate, and direct the nursing service department, along with its programs and activities, in compliance with current rules, regulations, and guidelines.
  • Prepare written and oral reports/recommendations to the ADHS/DHS as necessary regarding the operation of the nursing service.
  • Ensure the accuracy of census events in the Electronic Health Record.
  • Complete the scanning and attachment of resident documentation in MatrixCare to maintain comprehensive Medical Records.
  • Conduct admission assessments for all new residents and review findings with the primary nurse.
  • Assist in event investigations as directed by ED/DHS/ADHS.
  • Complete nursing admission assessments and baseline care planning for new residents.
  • Conduct MDS section GG Functional assessments as required.
  • Initiate and oversee restorative programs to ensure the provision of necessary services.
  • Ensure that appropriate daily skilled documentation is assigned and completed.
  • Complete notices of transfer and discharge, close discharged resident events, and ensure discharge orders are obtained and documented in the medical record.

Knowledge, Skills, and Abilities:

  • Ability to read, write, and follow written and oral instructions effectively.
  • Capacity to make independent decisions regarding the aforementioned job duties.
  • Ability to interact tactfully with personnel, residents, family members, visitors, government agencies/personnel, vendors, and the general public.
  • Knowledgeable in nursing and medical practices and procedures, as well as laws, regulations, and guidelines relevant to long-term care.
Qualifications

Must possess, at a minimum, a Nursing Degree from an accredited institution.

Licenses/Certifications

  • Must hold and maintain a current, valid state RN/LPN license.
  • Current, valid CPR certification is required.
  • Wound Care Certification (WCC) is preferred.
  • Infection Preventionist certification is advantageous.
