Hospice Care Planner

Responsibilities:
* Provide leadership guidance to professional staff
* Ensure provision of Hospice and Palliative Care services aligns with program goals
* Mentor and direct professional staff as directed by the Hospice Nurse Manager
* Function in a backup On-Call capacity 2-3 times per month and 2-3 holidays per year
* Collaborate with multidisciplinary teams to deliver quality care
* Utilize effective communication and organizational skills
* Uphold ethical standards and regulatory compliance
Requirements:
* Graduate from an accredited School of Nursing; BSN or BS degree preferred
* Minimum five years of Hospice nursing experience
* Supervisory or leadership experience preferred
* Proficiency in Word, Outlook, and Windows; New York State RN licensure required
