Responsibilities

The Hospice Care Planner will work on-site at a major medical center, evaluating and establishing a plan of care for patients referred for Hospice services. This role involves facilitating admission of referrals upon discharge from the hospital to a hospice program that meets the needs of the patient and family members. The successful candidate will have strong clinical experience and excellent communication skills to develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center.

The Hospice Care Planner will also be responsible for developing and maintaining relationships with home care, nursing, and other healthcare professionals to ensure seamless care coordination.

Qualifications
  • Licensed Registered Professional Nurse in NYS with current registration
  • Bachelor's Degree preferred
  • Minimum of two years experience in home care nursing with proficiency in physical assessment and interviewing skills
  • Excellent written and verbal communication skills, with a focus on client service
  • Advanced critical thinking and problem-solving skills, with the ability to work independently
Compensation

Per Diem Rate: $55/hour (Weekdays), $60/hour (Weekends)
