We are seeking a skilled RN Hospice Care Planner to join our team in Brooklyn or Manhattan. As a key member of our multidisciplinary team, you will provide leadership guidance to professional staff and ensure the provision of Hospice and Palliative Care services aligns with program goals.
Responsibilities- Provide leadership guidance to professional staff
- Ensure provision of Hospice and Palliative Care services aligns with program goals
- Mentor and direct professional staff as directed by the Hospice Nurse Manager
- Function in a backup On-Call capacity 2-3 times per month and 2-3 holidays per year
- Collaborate with multidisciplinary teams to deliver quality care
- Utilize effective communication and organizational skills
- Uphold ethical standards and regulatory compliance
- Graduate from an accredited School of Nursing; BSN or BS degree preferred
- Minimum five years of Hospice nursing experience
- Supervisory or leadership experience preferred
- Proficiency in Word, Outlook, and Windows; New York State RN licensure required
