Job Summary

At MJHS, we are seeking a highly skilled Hospice Care Planner to join our team. As a key member of our business development and marketing team, the successful candidate will be responsible for evaluating and establishing a plan of care for patients referred for Hospice services.

Key Responsibilities:

  • Evaluate patient referrals and develop a plan of care in collaboration with the healthcare team
  • Facilitate admission of referrals to a hospice program that meets the needs of the patient and family members
  • Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center
  • Collaborate with the business development and marketing team to identify opportunities for growth and expansion of our Hospice services
  • Provide clinical expertise and guidance to patients, families, and healthcare providers

Requirements:

  • Licensed Registered Professional Nurse in NYS with current registration
  • Bachelor's Degree in Nursing or related field
  • Minimum of two years experience in home care nursing with proficiency in physical assessment and interviewing skills
  • Excellent written and verbal communication skills
  • Advanced critical thinking and problem-solving skills, with the ability to work independently

Compensation:

Per Diem Rate: $55/hour (Weekdays), $60/hour (Weekends)
