About the Role

We are seeking a highly skilled Licensed Clinical Social Worker to join our team at Valor Healthcare. As a key member of our Behavioral Health team, you will provide essential services to our patients, including psychosocial assessments, treatment planning, and case management.

Key Responsibilities
  • Conduct thorough psychosocial assessments to identify patient needs and develop effective treatment plans
  • Provide short-term, solution-focused counseling to patients and their families
  • Collaborate with the PACT Team to develop and implement patient care plans
  • Develop and lead group activities to enhance social development and promote group goals
  • Provide education and referral for resources and benefits to patients and their families
Requirements
  • Master's degree in Social Work from a program registered by the Department of Education
  • Valid, unrestricted LCSW/MSW license in the state of desired employment
  • Minimum of 2+ years' post-master direct counseling experience with adult patients
  • Proven mastery of advanced sociological, psychological, and mental health care related interventions and issues
  • Certification in Basic Life Support and ACLS
