About the Role

We are seeking a highly skilled and compassionate Occupational Therapist to join our team at VitalCaring. As a key member of our home health care team, you will play a vital role in delivering high-quality care to our patients in the comfort of their own homes.

Key Responsibilities
  • Evaluate and assess patients to determine their suitability for home health care
  • Develop and implement personalized care plans to meet the unique needs of each patient
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care delivery
  • Deliver high-quality skilled care and supervise the care of other team members
  • Thoroughly document care delivery in our electronic medical record system
  • Contribute to a culture of excellence and compassion through individual accountability and teamwork
Requirements
  • Current Occupational Therapist License and valid state driver's license
  • One year of experience as an Occupational Therapist in an acute care, rehabilitation, or home health setting preferred
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Compassionate and patient-centered approach to care delivery
