About the Role

The Case Manager provides critical services in our Guardianship program, supporting the coordination of direct services for 35-40 wards. This includes placement, monitoring of care provided, facilitating medical, social, psychiatric, and financial needs.

Key Responsibilities
  • Visit clients at least once monthly and monitor the quality of care delivered.
  • Prepare comprehensive initial and annual reports for the court, ensuring accuracy and timeliness.
  • Maintain client files and case notes, documenting all interactions and progress.
  • Manage assets, handle banking tasks, and facilitate benefit applications on behalf of clients as needed.
  • Conduct case conferences with the Program Director, Client Assets Manager, and other caseworkers as necessary.
  • Cultivate relationships with volunteers and coordinate with professionals involved in the continuum of care.
  • Assume 24-hour emergency call duties, shared with the Program Director and other caseworkers.

We offer a competitive salary range of $60,000 - $75,000 per annum, depending on experience.

Qualifications and Requirements

To be successful in this role, you will require:

  • Bachelor's degree in Social Services, Accounting, or a related field.
  • Experience in guardianship is preferred, but not essential.
  • A background in social services, medical nursing, law, insurance, or finance is beneficial.
  • Knowledge of community resources and services is advantageous.
