Job Description:

Role Overview

As a Patient Care Coordinator at Peoples Home Health, LLC, you will play a vital role in ensuring our patients receive high-quality care and support. Your primary responsibilities will include preparing social histories, care plans, and providing social casework to patients and families experiencing difficulties in social functioning.

You will work closely with interdisciplinary teams to assess clients' psychosocial status, identify strengths and weaknesses, and develop effective care plans. Additionally, you will participate in case conferences, referrals to community agencies, and maintain accurate records and reports.

Key Responsibilities
  • Prepare social histories, care plans, and provide social casework to patients and families
  • Assess clients' psychosocial status and identify strengths and weaknesses
  • Develop and implement effective care plans in collaboration with interdisciplinary teams
  • Participate in case conferences and referrals to community agencies
  • Maintain accurate records and reports
Requirements

To be successful in this role, you must possess a Master's degree in Social Work and have 1-2 years of experience in a home health agency or hospital setting. You should also hold a current driver's license with access to an auto and required insurance coverage. CPR certification is a plus.

We are looking for a self-starter who can take initiative, identify needs of co-workers and customers, and solve problems effectively. You should possess strong reading comprehension, time management, speaking, writing, and critical thinking skills.
