Position Title: Master's Social Worker (LLMSW and LMSW)
POSITION SUMMARY:
Under the guidance of the Social Work Manager, the Master's Social Worker (MSW) operates autonomously to evaluate the requirements of, coordinate, and provide social services for participants and their families or caregivers. The MSW is an integral member of the Participant Care Team (PCT) and the Interdisciplinary Team (IDT), engaging in the assessment, intervention, management, and review of social service necessities. The MSW aids the IDT in assessing eligibility and care requirements. Care is delivered through various modalities, including but not limited to, comprehensive bio-psychosocial assessments, individual and group counseling utilizing motivational interviewing and cognitive behavioral techniques, psychoeducation, consultation, and care planning/case management.
KEY RESPONSIBILITIES:
- The MSW is part of the PCT and IDT, conducting thorough bio-psychosocial assessments upon enrollment and at regular intervals thereafter. Acute assessments are performed as needed to address specific referral inquiries, employing measurement-based care.
- Creates and executes tailored treatment plans for both participants and caregivers, which may include:
- Individual and/or group counseling (utilizing Motivational Interviewing and CBT techniques)
- Educational initiatives
- Consultation services to the Care Team
- Risk Assessment and Crisis Intervention
- Manages case management issues, which encompass:
- Housing challenges
- Financial concerns
- Safety matters, including signs of abuse, neglect, or exploitation; ability to live independently; decision-making capacity; and any issues necessitating involvement of Adult Protective Services (APS)
- Psychiatric evaluations: Changes in mood, behavior, cognition/mental status; suicidal/homicidal ideation; substance use/misuse
- Caregiver stress
- Identifying the need for transfer to a higher level of care (e.g., Behavioral Health Services).
- Facilitates discharge planning activities and processes.
- Coordinates 24-hour care as indicated by the interdisciplinary team and/or the Care Team.
- Educates and advises the participant and/or caregiver regarding care needs, options, and related issues.
- Advocates on behalf of the participant, demonstrating accountability in resolving participant concerns or issues.
- Collaborates to develop and communicate initial impressions, care plans, changes in care, and progress in a timely manner.
- Assists in formulating and finalizing the care plan with the IDT, participant, and/or caregiver.
- Ensures timely and accurate documentation in the electronic health record (EHR).
- Communicates changes in participant status with IDT and family members.
- Evaluates participant outcomes and progress toward achieving the objectives and goals of the care plan, sharing this information among other members of the PCT and IDT.
- Performs additional related duties as assigned by leadership.
QUALIFICATIONS:
- Master's degree in Social Work from an accredited institution.
- Licensed to practice in the State of Michigan. Limited License may be considered based on experience and supervised hours.
- A minimum of one year of experience as a clinical social worker, preferably in geriatrics. Knowledge of human behavior principles, individual and group therapy techniques (preferably CBT and DBT), short-term interventions, and substance use disorder assessment and treatment.
- Demonstrated ability in evaluating, planning, implementing, and monitoring patient-centered care approaches.
- Familiarity with applicable local, state, and federal laws and regulations.
- Cultural competence and ability to serve a diverse population.
- Proficient in using electronic health record systems.
- Skilled in Microsoft Word, Excel, Outlook, and PowerPoint.
- Possesses a positive, flexible, and solution-oriented attitude with the ability to collaborate effectively in a team environment.
