Position: Social Worker Intake Coordinator

Role Summary: The Social Worker Intake Coordinator at PACE Southeast Michigan is integral to the initial assessment process for prospective program participants. This position involves conducting home evaluations, determining eligibility, and delivering vital information regarding the program to individuals and their families.

Key Responsibilities:

  • Perform comprehensive Social Work evaluations to assess program eligibility
  • Execute all components of the Home Visit protocol
  • Maintain effective communication with the Intake team and provide ongoing updates
  • Enter participant data into the management system
  • Collaborate with eligibility specialists to navigate Medicaid and Medicare criteria
  • Follow up with potential participants and their families as necessary
  • Ensure the confidentiality of participant records

Qualifications:

  • Master's degree in Social Work
  • Possession of a Limited or Full social work license
  • Experience in community-based services for the elderly
  • Familiarity with resources available for senior citizens
  • Proficiency in Microsoft Windows and Outlook
  • Excellent interpersonal and communication abilities
  • Capability to effectively promote the program
  • Valid driver's license for travel within designated areas
