Job Summary

The Patient Aligned Care Team (PACT) Licensed Practical Nurse (LPN) plays a vital role in providing high-quality patient care in the Primary Care setting. As a member of the PACT team, you will work collaboratively with healthcare professionals to deliver comprehensive care to adult and geriatric patients.

Key Responsibilities
  • Conduct thorough nursing intakes and provide individualized care to patients based on their unique needs and circumstances.
  • Collaborate with the PACT team to develop and implement comprehensive health care plans and care management strategies.
  • Provide clinical support and guidance to patients and their families, promoting health education and self-management skills.
  • Participate in performance improvement activities to optimize team efficiency and patient care delivery.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions and coordination.
Requirements
  • Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or the Accreditation Commission for Education in Nursing.
  • Full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare team members.
